Olivia is a 4 year old Caucasian female that’s lives with her mom, dad, 8 y/o and 1 y/o brothers. Both parents smoke in the home. Olivia attends preschool 5 days a week. All her immunizations are up to date. Her mother is a nurse and has been treating her with Children’s Tylenol, saline nasal spray and humidifier at night. Exam: Vital Signs T-38.5, P-108, RR-26, BP-88/50, weight- 37lbs, height-40”
General: She is active, alert and in no apparent distress, HEENT: right tympanic membrane is erythematous and bulging with poor mobility and decreased light reflex, left tympanic membrane is clear with good mobility. Throat is non-erythematous. Lymph: There a small cervical lymph nodes palpable. Lungs: Clear to auscultation. The rest of her exam is normal. She has no known drug or food allergies
