Summarize a patient’s geriatric and functional health assessment
Purpose: Perform a Functional Assessment on an older adult to determine their functional status.
Directions: (In preparation for this assignment read page 56 and Chapter 31 in Jarvis)
- Apply the following Functional Assessment tools identified below on someone over the age of 50.
- Write a paper in a paragraph format that is between 1- 2 pages. Include the following sections: (prior to each section create headings with the highlighted concepts below– do not include lists or bullets. Include a cover page.
- Sections:
- Health History: Brief health history to include age, gender, brief social history (marital status, occupation, use of alcohol, tobacco, other substances), medications (just the names – no dosages, etc.), and past medical history). (No review of systems).
- Rationale for Each Functional Assessment Tool: Provide a brief summary about what each tool is assessing.
- Results of the Functional Assessment Tools. Summarize the client’s results from each of three assessment tools. Specifically include the final score and what that indicates. (Do not include all the survey items and individual answers)
- Over All Functional Ability: Summarize the client’s overall functional ability based upon the results of all three survey tools.The summary will include the client’s strength and weaknesses related to his/her ability to perform ADL’s and IADL’s. Specifically identify the ADL’s and IADL’s you are referring to. * (Prepare for in class discussion- see criteria below)
- Paper Organization: Utilize APA format- include in text citations. Keep in mind when you write about the different scales from the textbook it is a source to be cited. Include a reference page (even though you only have that 1 reference).Paragraphs are directly related to the sections in the directions.Grammar is clear and logical.
